Evidence
File evidence about a certified entity
If you believe a certified company is not meeting the commitments it made, tell us. Submissions may trigger review under the Revocation Protocol.
Three ways to raise a concern
01
Rights-holders - complaint
Report conduct that may breach a certified entity's published governance commitments.
02
Regulators - referral
A channel for competent authorities. Referrals are treated as priority.
03
Compliance - confidential
Current or former staff. You can request confidential handling of your identity. The Confidentiality Protocol explains the limits.
WHO CAN FILE
WHAT WE CAN ACT ONAI:OK is a voluntary certification scheme. We can issue, maintain, suspend and withdraw our own trustmark - and we publish every status change on the registry.
We cannot award damages, compel a company to act, or enforce findings against anyone. If you need those remedies, a regulator or a court is the right route, and you can pursue both at once. We can only act on companies AI:OK has certified.

FAQs
What happens nextWe acknowledge every submission within five working days and carry out a preliminary review. Where a matter proceeds, the certified entity is notified with the specific ground and given a reasonable opportunity to respond before any published status change.
What this is notFiling here does not create a legal obligation on AI:OK to act, and it does not replace legal action or a complaint to a competent regulator. Where both are appropriate, pursue both. AI:OK is a voluntary certification scheme.
ConfidentialitySubmissions are handled under the AI:OK Confidentiality Protocol. Access is restricted to those who need it to handle the matter. This may include assigned assessors, panel members and authorised technical administrators, as explained in the Confidentiality Protocol. We will not share it with the certified entity beyond what fair process requires, or with any other party except where required by law.
